Comparative Review of Herbal Vs Conventional Therapy in Type 2 Diabetes Mellitus

Abstract

Type 2 diabetes is a growing health problem in Pakistan, causing a big economic and healthcare burden. Conventional anti-diabetic drugs such as Metformin remain first-line therapy; however, interest in traditional unani and ayurvedic medicinal plants including Fenugreek (Trigonella foenum-graecum) and Berberine-containing herbs has increased because of their affordability, cultural acceptance, and perceived safety. This review compares fenugreek, berberine and metformin for controlling blood sugar levels, reducing HbA1c , risk of developing hypoglycemia, cost, and how well patients follow treatment. Evidence suggests that metformin works best and consistently reduces HbA1c (1.0%-1.5%). Fenugreek (0.5%-1.0%) and berberine (0.7%-1.2%) shows modest reduction in HbA1c. Herbal treatments usually have a low risk of hypoglycemia and are less expensive but have issues with the standardisation, varying dosages, and limited long term evidences. In Pakistan, socioeconomic factors, access to remedies and medicine affordability greatly affect how well patients follow their treatment. Combining conventional medicines with regular diabetes care may improve access to treatment and patient care. Type 2 diabetes mellitus (T2DM) and diabetes management are major growing issues in Pakistan. Integration of evidence-based herbal medicine with conventional diabetes management may improve accessibility and patient-centered care.
